How To Create & Edit Questionnaires
Find out how to create and edit each type of questionnaire.

📝How To Create A Standard Questionnaire
This type of questionnaire is built with questions in the order that you would like the candidate to answer them in.
The set up will be in a specific order to how you have created it and candidates will need to answer all of these.
➡️Getting Started
To begin creating a questionnaire, click on the + symbol at the top of the screen and select “Standard Questionnaire”.
You will then be taken to the next screen where you will be able to give your questionnaire a name, a description and select the questionnaire type from a drop down list.

There are several options of questionnaire that you can select and create from the drop down list, which you can see below:
- Pre-screening
- Equal Opportunities
- Campaign Planning
- Shortlist
- CV Feedback
- Interview feedback
- Offer
- Other (Previously used for Diversity)
- Onboarding
- CRM (Campaigns)
Once you have decided what questionnaire type you would like to set up, you can then begin to add your pre-written questions, which can be added using the +AddQuestion button situated on the bottom right.
After clicking on the +AddQuestion button you will see the “Question Bank” and will be able to either search for a specific question or scroll through the list and select one or more by clicking the small box on the left of each question and then pressing “Insert”.
For this type of questionnaire, you can select as many questions at the same time as you want to add.
Once you have all of your desired questions in place you can see how your questionnaire will be presented and the current order of questions and edit the validation if so desired.
➡️ How To Minimise Questions
If you have selected a lot of questions and want to make the view easier for you, you can select the multiple arrows at the side of your question. This will minimise the question and answer so it's easier to view whilst you are building.
➡️ How To Delete A Question From Your Questionnaire
If you want to amend and delete a question you can press the X which is next to the triple arrows to the right of the question. This will remove the question from the questionnaire.
➡️ How To Move A Question
If you need to move a question to a different order, all you need to do is hover over the question that you want to move and you will notice dotted blue lines around the edge show up. If you hold your cursor down a “crosshair” will then appear allowing you to drag and drop to move the question anywhere you wish within the Questionnaire.
Once you are happy with your questionnaire simply click the Finish button.
Your questionnaire is now ready to assign to job roles.
🌳How To Create A Branching Questionnaire
This type of questionnaire allows you to link your questions together. This in turn allows your candidates to answer the branch of a questionnaire dependent on their answer. Two candidates answering the same questionnaires may have different routes depending on how they answer.

You will then be taken to the next screen where you will be able to give your questionnaire a name, a description and select the questionnaire type from a drop down list.

➡️ How to branch questions together
Once you have inserted a question you will see the option to "🔗link to question" situated at the bottom of the question.
If the candidate selects yes, generally we want them to answer more about the specific question. So in this instance we would select the "yes" variable and choose another qualification question and select “Link Questions”.
If the candidate selects yes, they will then answer the above question. However, if the candidate selects no, we can ask them a very different question.
Now you will see that both answers are now linked to questions:
Depending on how the candidate chooses to answer, will determine the different branch of the questionnaire that they will be taken down.
➡️ How To Minimise Questions

➡️ Questions That Are Not Fully Linked
If at any point in the questionnaire there is a break and there is no "link" to the next question, the next question will NOT be answered by any candidate.
All questions must be linked together to ensure that the full questionnaire is answered.

If you need to move a question to a different order, all you need to do is hover over the question that you want to move and you will notice dotted blue lines around the edge show up. 📚How To Create A Hybrid Questionnaire
This type of questionnaire allows you to use a combination of a Standard and Branching questionnaire. This in turn allows your candidates to answer questions in a linear order as well as answer the branch of a questionnaire dependent on their answer.

To begin creating a hybrid questionnaire, click on the + symbol at the top of the screen and select “Branching Questionnaire”.
You will then be taken to the next screen where you will be able to give your questionnaire a name, a description and select the questionnaire type from a drop down list.

From this point forward you will need to follow the steps on how to add questions from our ⚓ creating a standard questionnaire and ⚓ creating a branching questionnaire.
🖊️How To Edit A Questionnaire
In a similar fashion to making changes to a question you also have the option to amend your questionnaires.
To get started head over to the tools tab in the menu bar and select “Questionnaires”. Once you have clicked into the questionnaires tab you will be presented with the list of previously created questionnaires.
In order to find the questionnaire you are wanting to edit, either use the search bar function in the top right and type in the questionnaire title or scroll down to find the relevant questionnaire.
When you have found the questionnaire that you are wanting to amend, you will have the option to:
- Hide It Or Make It Visible
- Delete It
- Edit It
If you would like to hide your questionnaire click on the toggle to the left.
If the toggle is pushed to the right then the questionnaire is visible to select or visible on a job.
If you move it to the left it will hide the questionnaire and it will no longer be available to new candidate applications.
Or if you would like to delete this questionnaire click on the bin icon instead.
And finally If you would like to begin editing your questionnaire simply click the pencil icon.
After clicking on the pencil icon, you will be taken to the questionnaire builder.
Once you are in the questionnaire builder you have the option to do the following:
- Clone the questionnaire
- Amend the questionnaire's name
- Add a new or different questionnaire description
- Change the questionnaire type
- Add in or change the organisation
- Cascade this questionnaire to branch
- Amend any applicant threshold scoring
- Add or Remove a question
If you would like to add a question, click on the +add question button at the bottom of the screen and follow any previous steps you followed when setting your questionnaire up.
If you would like to remove a question simply click the X in the top right corner of the question you want to delete.
There is also functionality to hide a question if you don't want to delete it from the questionnaire, to hide the question you need to select the upward arrows next to the X on the right.
The question will be visible to you in the questionnaire but not to the candidate when completing it. To un-hide the question simply click on the downward arrows and it will become visible again.
🏣 Assigning Hierarchy To A Questionnaire
Hierarchy is used to reflect the internal structure of your organisation, allowing you to restrict job visibility based on what a user should see when they access the ATS.
If your organisation uses questionnaires you can align hierarchy according to who should be able to use and see the questionnaire itself.
This is very useful if you have a long list of questionnaires to select from, meaning that users will only see what they need to see.
To do this, you will either need to be creating a questionnaire from scratch or editing a previously created questionnaire.
In the questionnaire builder, you will see a menu called 'Organisation' and select the field after doing so the hierarchy will appear.
From here you can select the relevant hierarchy node/s, and will be able to select more than one if needed.
Below Organisation, you will see the option to: 'Cascade to branch'.
If you select a top level from within your hierarchy (rather than individual levels), you can choose yes to cascade visibility of the questionnaire to all levels that fall below it.
In the example below, Region Europe has been selected, which means any users aligned to the UK and Germany will also have visibility of the questionnaire.
Once you are happy with your questionnaire simply click the Finish button.
